Service Description

24-Hr Awake Service - Graham Shelter is a 24 hour, three shift wake group home for adolescent males. Supervision is documented by a daily log book that is completed by 1st, 2nd and 3rd shift.

24-Hr Emergency Service - Graham Shelter provides 24 hour care which would provide the proper treatment and transportation that would be required in the event of an emergency. Services include 24 hour supervision, mentoring, school transportation, counseling transportation, medication management, sleeping quarters and meals.

 

Service Description

Attendance at Court Hearings - Graham Shelter provides a staff member at any and all court hearings regarding the client.

Behavior Modification - Graham Shelter provides a list of points and levels a client can earn on a weekly basis as a measurement of behavior modification. A client can earn levels to gain privileges or lose privileges with loss of levels. Clients are encouraged to maintain levels for approved passes, outings, privileges and a positive recommendation for discharge when available.

Big Brother/Big Sister - Graham Shelter provides the opportunity for a client to become involved in the Big Brother/Big Sister program. The agency will be contacted at the request of the client. Graham Shelter will make the necessary arrangements for client’s who already are participating in the program.

CASA Services - Graham Shelter encourages CASA involvement with caseworker approval. Graham Shelter corresponds with CASA with legal support for the client and possible client placement.

Case Management - Graham Shelter provides case management to the client such as treatment plans, treatment plan reviews, medical appointments, monthly logs and correspondence to their caseworker or probation officer.

Child Abuse Prevention - Graham Shelter provides safety and security to the best of its ability to ensure clients are safe and secure at all times.

Clergy Services - Graham Shelter provides transportation to and from clients’ denomination of religion. Weekly groups are also provided for those who are interested.

 

Concrete

Clothing - Graham Shelter provides a clothing checklist when a client arrives at the facility and provides them with adequate seasonal clothing throughout the year. A clothing inventory is then taken when the client is discharged.

Emergency help - Graham Shelter provides 24 hour care which would provide the proper treatment and transportation that would be required in the case of an emergency.

Food - Graham Shelter provides three daily meals that are approved by the state dietician with snacks provided in between the meals.

 

Counseling

Outpatient services - Graham Shelter provides transportation to local counseling agencies. Staff also provides information and updates on the client at the request of the therapist. Some local counseling agencies include Park Center Inc, Bowen Center, Mcgee, Lytle and Associates, Phoenix Associates and other agencies referred by caseworkers, probation officers, court officials and parents. Some examples of therapy include individual, group, sexual abuse, drug and alcohol, crisis or other issues that client needs to focus on. Graham Shelter provides transportation, encouragement and correspondence with treatment team that could assist with positive interventions.

Community Assistance - Graham Shelter provides assistance to local programs that need assistance for events. This would include supervised volunteering, fund-raising and community events for client’s who would be interested in helping.  

Community Awareness - Graham Shelter provides access to organizations and outings that focus on community awareness such as Fort Wayne Urban League. Some outings include St. Mary’s Soup Kitchen, Habitat for Humanity and Fort Wayne Rescue Mission.

Developmental Disability

Daily Living Skills Training - Graham Shelter staff provides assistance and coaching for daily living skills.

Self Care Training - Graham Shelter staff provides assistance and coaching for self care training.

The following developmental disability services are provided by an outside agency with staff assistance:  Other Dev. Disabilities Service, At Risk Services, Child Dev. Assessments, First Steps Program, Diagnostic/Evaluation/Assessment Services, Domestic Violence Services (Treatment), Dual Diagnosis Program

 

Educational Services

After School Programming - Graham Shelter provides transportation and encouragement for those who are eligible to participate in after school programming. After school programming might include sporting events, assisted tutoring, club interests and other programming that a client might be interested or eligible for.

Educational Planning - Graham Shelter assists client with guidance and encouragement to take the appropriate classes for grade advancement or graduation.

GED- Graham Shelter provides resources and transportation to and from GED programs as well as tutoring.

Study Skills Group - Graham Shelter provides daily study skills group with book and computer resources as well as staff assistance.

Supervised Study - Graham Shelter study tables are monitored by staff as well as a tutor that is on site four days a week.

The following developmental disability services are provided by the local public school system and outside agencies with staff assistance:  Academic Credit Courses, Accredited School Program, Adult Education, Testing, Tutoring (Remedial Services), Even Start Program, Family Planning Services, Family Preservation Services, Family Support Worker, Alternative School, Assessment, Continued Ed./Ed. Independent Living

Mentoring - Graham Shelter provides in house and community resources with staff assistance for knowledge.

Daily Living - Graham Shelter provides discussion and demonstration by staff for daily living. Examples of daily living include hygiene, laundry, room care, daily chores.

Financial Assistance - Graham Shelter assists with saving, spending and budgeting money that a client would make through weekly allowance. Bank accounts are established for those who have part-time employment or those who wish to open a bank account. Assistance and information is given to questions about saving and budgeting money as well as bank accounts.

Job Placement - Assistance for employment opportunities and job coaching are offered to client’s who are eligible and willing to seek part-time employment.

Job Retention - Graham Shelter corresponds with supervisors for updates as well as encourages client to maintain employment. 

Other IL Skills - Graham Shelter provides resources and assistance that a client might be interested in other independent living skills.

Skills Training - Graham Shelter provides resources and discussion to develop skills that are needed on a daily basis.

Social Life Skills - Graham Shelter provides discussions, experiences and resources to assist with social life skills.

The following independent living services are provided by outside agencies with staff assistance:  Classes, Emancipation Services, Life Space Counseling, Transitional Living Services/Plans, Juvenile Offenders’ Program, Juvenile Work Program, Legal and Advocacy Services

 

Service Description

Long Term Residential Care: Graham Shelter is a long term care facility for adolescent males.

 

Medical

Emergency - Graham Shelter staff is certified in CPR and first aid for situations that require attention immediately before seeing a physician.

Medication Management - Graham Shelter ensures that all medication appointments are met and prescriptions are properly filled. Graham Shelter ensures that client is receiving the proper medication and dosages are given at the correct time.

Other medical - Graham Shelter ensures that all medical appointments such as dental, immunizations, tb tests, physicals and optometry appointments are up to date.

The following medical services are provided by outside physician agencies with staff assistance:  Blood Disorders Services, Cardiac Disorders Services, Examination, Facial Deformities Services, Health Care/Services, HIV Testing, Inpatient Psychiatric Hospital, Inpatient Treatment, Nursing Services, Physical Special Needs Services, Psychiatric (Eval., Serv., Treat), Psychotropic Medication, Seizure Services, Visiting Nurse Service, Mental Health Services, Mentor Services (Prog.-Group), Observation & Pre-assessment, Other Family Support Services, Other Special Needs Services

 

Leisure Activity

Leisure Education - Graham Shelter provides resources and transportation to activities, interests or hobbies that clients’ might have and request. Graham Shelter provides supervision, transportation and participation in local interests such as sporting events, library, zoo, parks, museums, movies, arcades, festivals as well as out of state events when approved by caseworkers or probation officers.  

Other Recreation - Graham Shelter provides supervision, transportation and participation in local interests such as sporting events, library, zoo, parks, museums, movies, arcades, festivals as well as out of state events when approved by caseworkers or probation officers.

Social Development - Graham Shelter provides the resources, confidence and encouragement to interact with others in their community. Clients are encouraged to participate in activities with others in the community. Participation in organized leagues to assist with developing character, teamwork and social skills.

 

Service Description

Transportation Services - Graham Shelter provides transportation to and from numerous locations. This includes appointments, school, activities, outings, passes and other necessary locations.

Visitation Services - Graham Shelter provides supervised and non-supervised areas in the group home for visits with family members and other approved people when approved by caseworkers, probation officer and court officials. Visitation is permissible Saturdays and Sundays from 10am-6pm.

 

Job Assistance

Job Assistance - Graham Shelter provides assistance for employment opportunities and job coaching is offered to client’s who are eligible and willing to seek part-time employment.

Volunteer Services - Graham Shelter provides assistance to local agencies, churches and fairs with option for volunteer services provided by clients with staff guidance and assistance.

 

Parent Education

The following parental education services are provided by outside agencies with staff assistance:  Classes, Homebased, Nurturing Program/Classes, Parenting Assessments/Evaluations, Parent Aide Services, Placement Retention, Polygraph Test

 

Pregnancy

The following pregnancy services are provided by outside agencies with staff assistance:  Teen Parent Prevention, Teen Parent Services, Psychological Evaluation, Psychological Testing

 

Psychological Education

The following psychological services are provided by outside agencies with staff assistance:  Anger Management, Behavior Management, Conflict Resolution, Parenting Skills, Relationships, Responsible Sexuality, Self-Esteem Enhancement

 

Sexual Abuse

The following sexual abuse services are provided by outside agencies with staff assistance:  Non-abusing Parent, Non-offenders, Offender Treatment Group (Adult/Child), Victims’ Treatment (Group, Non-Group), Services for the Mentally Retarded, Services to Locate Absent Parent, Relative, or Primary Caregiver

 

Support Groups

The following support group services are provided by outside agencies with staff assistance:  Bereavement, Devotional, Dissociative Disorders, Eating Disorders, Family Violence, Foster Children, Other Support Groups, Parents, Self Help, Teen Parent, Sheltered Workshop

 

Substance Abuse

The following substance abuse services are provided by outside agencies with staff assistance:  Aftercare, Evaluation/Assessment, Kids/Adults, Other Substance Abuse Services, Screens (Blood, Urine), Treatment (Outpatient, Inpatient)

 

Substance Abuse Support Groups
